To help alleviate confusion and stress, transportation assistance and mealtimes are managed, too. Three nutritious meals are served at the same time each day, and place settings are provided to make the dining experience even easier. To promote peace of mind, staff members are available 24 hours a day, seven days a week to be alerted to an emergency. Plus, keypad entries and a 24-hour security system are also used. Additionally, specialized features, including a large circular path, way-finding cues, and an enclosed garden and courtyard, also help to promote security. A Brookdale HealthPlus ® Community At Brookdale Creekside, residents can receive a higher level of care and the benefit of care coordination overseen by a registered nurse through the exclusive Brookdale HealthPlus program. The Brookdale Care Manager (RN) can serve as medical advocate for residents and assist with organizing care transitions, coordinating referrals, scheduling follow up visits, and more. Preventative healthcare services, such annual wellness visits, immunizations, and health screenings are provided, as well as help with managing chronic conditions. Residents with Alzheimer’s and other types of dementia have access to two revolutionary memory care programs — Clare Bridge and Solace — which provide gentle guidance and daily reminders. The Clare Bridge program provides residents with at least six activities each day to encourage the use of their current skills and abilities. Meanwhile, the Solace program offers support to residents in the advanced stages of dementia and allows time for additional grooming assistance and sensory-based activities using music and aromatherapy. Community Services and Amenities While your chores and care are managed, you can focus on the hobbies or activities you love most. In the state of Texas, these terms are defined as follows: Assisted Living Facility A facility that provides personal care services and administration of medication to 4 or more residents. Assisted living facilities can accept residents with dementia and/or Alzheimer’s. There are 3 types of assisted living facilities: Type A: A facility that may provide room and board to residents who are physically and mentally capable of evacuating the facility unassisted. Type B: A facility that may provide room and board to residents who need assistance evacuating, are incapable of following directions under emergency conditions, require staff attendance during nighttime sleeping hours, and require assistance transferring to and from a wheelchair. Type C (Adult Foster Care): Four-bed facilities that meet the minimum standards and rules for enrollment in the Department of Aging and Disability Services’ Adult Foster Care home program.
