A Brookdale HealthPlus Community ® The residents at Brookdale Skyline can enjoy the benefits of Brookdale HealthPlus, which offers on-site care coordination overseen by a registered nurse. Here, residents receive personalized care plans and assistance with chronic conditions, such as diabetes, high blood pressure, congestive heart failure, and more. Preventative health care services, such as annual wellness visits, immunizations, and health screenings are also provided to help avoid trips to doctors’ offices, or unnecessary emergency room visits and hospital stays. Care Types Offered Brookdale Skyline is an assisted living residence , which in the state of Colorado is a residential facility which provides personal services, protective oversight, social care due to impaired capacity to live independently, and 24 hour supervision (but not to the extent that regular 24-hour medical nursing care is required). Assisted living residences may also provide care to no more than 16 elderly persons who are mentally ill.
