Quality compassionate care for patients and their families

Caring staff have done a good job of dealing with my mom and her dementia, and keeping my mom safe. I have not had any trouble getting updates about my mom's condition. Staff have pretty much always been available to answer questions for both me and my mom at all hours. The place is clean, the food is good, and the staff have been caring people to both me and my mom. Though my mom has often opted out, the variety of activities at Ivy Park San Tomas is good, and occasional field trips make for good stimulation and v

The facility is clean with good food. However the nursing staff leaves much to be desired. They will not help with OTC meds easily and coordinating doctors visits can be tricky.

Give it a year or two before placing your LO here

The room is tiny. The meals were not as promised for almost a month. The activities were non existent for almost a month. There was no way for her to reach staff for several days after moving in. The staff isn't very attentive. The leadership gives good lip service but everyone is new and isn't aligned. The website is contradictory to the contract in several areas. We still haven't been able to get her hair done. There is a lot.

Response from the community

I would like the opportunity to speak with you directly—please call me at 669-201-2015 or stop by my office so we can discuss this further. Sincerely, Karen Nickolai Executive Director

Review for Ivy Park at San Tomas

First, the facility is great. Small, only around 50 residents (not full yet) on 2 floors. Well appointed. Bright colors. Extra wide hallways. Beautiful outdoor patio on the 2nd floor. Residents can go in on own, but exit through the dining/activity area and it is enclosed in glass so residents can be observed. With only 25 or so rooms on each floor, the design is simple, no long walks for residents. Rooms are very nice, Mom has a single. Pretty spacious. Large picture window for natural light & ability to open wind

Response from the community

Thank you for sharing such thoughtful feedback. We truly appreciate your kind words about the design of our community, the apartments, dining, and most importantly the care your mom is receiving. It means so much to know she is comfortable, engaged, and well supported. We’re also grateful for your patience during a time of leadership transition and are encouraged by the stability now in place. Knowing that you feel optimistic about the future and confident in our care team means a great deal to us. Thank you again for your trust and for being part of our community.

Need to be Prepared

Today was worst experience yet. I arrived at 8 AM to visit my brother and he was still in his room. Staff was short due to employee illnesses. It was still bad They had maintenance staff helping get some of the patients out of their rooms at 10:30 AM an several of them hadn’t had breakfast yet. They need to be better prepared for these circumstances.
