CriticalWe had to move mom because we felt she wasn't getting the care that was promised when we admitted her and that staff turnover was a big problem. We had to find another facility because we were told mom need to much care while she was in their skilled care facility.

When we first moved my loved one into the facility, it was agreed that the on staff nurse at Oak Park Place would treat my loved ones wound once a week and a grace staff would treat it twice a week because my loved ones doctor ordered it and required it to be treated three times a week for their ongoing health. CriticalOur loved ones room receives the 1 out of 5 for cleanliness, but the best part about the activities is the music. My Dad was there for 3 years and we only had one formal review of his status and care. My understanding is we were to have them every 6 months. Staff changed so often we never got to know any of them.

CriticalThe entire experience was overwhelmingly disheartening. My father in law did not get the care that was sold to us. While there were a few wonderful employees, none of them had the resources needed to do their job well. As I nurse, I was shocked by the quality of the care. I would not recommend OPP to anyone.
