Oak Park Place Madison West Nakoma

Very disapointing facility

We had to move mom because we felt she wasn't getting the care that was promised when we admitted her and that staff turnover was a big problem. We had to find another facility because we were told mom need to much care while she was in their skilled care facility.

My tour guide, [name removed] was outstanding. You can tell that he is proud to represent Oak Park Place and that he genuinely cares about its residents. He is knowledgeable, very patient and was very empathetic when answering questions pertaining to my dad’ s care. Oak Park Place is fortunate to have [name removed] on the frontline for them.

Overall a nice place to live.

They don't have enough staff, but that is true of all assisted living facilities in the area. Other than that, it has turned out to be a nice place for her to live. They need more variety on the meu. Also, they ask residents what they want from a list, but sometimes they get something different.

The common areas of the community are very clean but our loved ones individual room would get a 1. When we first moved my loved one into the facility, it was agreed that the on staff nurse at Oak Park Place would treat my loved ones wound once a week and a grace staff would treat it twice a week because my loved ones doctor ordered it and required it to be treated three times a week for their ongoing health. 'Room for Improvement

Our loved ones room receives the 1 out of 5 for cleanliness, but the best part about the activities is the music. My Dad was there for 3 years and we only had one formal review of his status and care. My understanding is we were to have them every 6 months. Staff changed so often we never got to know any of them.

Would not recommend

The entire experience was overwhelmingly disheartening. My father in law did not get the care that was sold to us. While there were a few wonderful employees, none of them had the resources needed to do their job well. As I nurse, I was shocked by the quality of the care. I would not recommend OPP to anyone.
