Residents’ benefit from care coordination overseen by a Brookdale HealthPlus Care Manager (RN). Brookdale’s memory care programs encompass a person-centered approach to care. The two programs offered are the Clare Bridge and Solace programs. The Clare Bridge program is an exclusive brand of dementia care which was created more than 30 years ago, and continually evolves with research. Clare Bridge provides residents with six planned activities each day to promote the use of their current skills and abilities. The Solace program includes similar elements, but offers support to residents in the later stages of dementia — it provides assistance with grooming, as well as time for musical and aromatherapy activities. Visual reminders and guidance are provided through daily activities like kitchen work, baking demonstrations, and other familiar activities to help residents flourish and maintain meaningful life skills.
