A Brookdale HealthPlus Community ® The residents at Brookdale Dr. Phillips (MC) can enjoy the benefits of Brookdale HealthPlus, which offers on-site care coordination overseen by a registered nurse. Here, residents receive personalized care plans and assistance with chronic conditions, such as diabetes, high blood pressure, congestive heart failure, and more. Preventative health care services, such as annual wellness visits, immunizations, and health screenings are also provided to help avoid trips to doctors’ offices, or unnecessary emergency room visits and hospital stays. Brookdale Dr. Phillips offers residents a person-centered approach to memory care. Here, each individual receives a personalized care plan to enhance their experience and quality of life. Brookdale offers two memory care programs which cater to those in the early, mid, and late stages of dementia. Although it was created more than 30 years ago, the Clare Bridge program continually evolves with new research. The program provides residents with six planned activities per day. These activities encourage residents to use their current skills, while preserving their abilities and true sense of self. The Solace program does not replace any elements of Clare Bridge. Instead, it adds additional programming, such as music and aromatherapy, while offering assistance with managing medications, dressing, and more. To provide residents with structure and stability, Brookdle’s team works to understand each resident's history, routines, and needs. In addition to providing custom care, the staff assists with cooking, cleaning, lawn care, transportation, and other tedious chores. Rest assured, dementia-trained team members are available 24 hours a day, seven days a week to be alerted to an emergency. Additionally, residents benefit from other safety features, including an enclosed courtyard, secured entrances and exits, keypad entries, and an emergency alert system. Residents also have access to an on-site pharmacy as well as a beauty and barber shop. The goal here is to make everyone feel safe and supported. Community Services and Amenities Residents can enjoy morning brain games, flower-arranging workshops, popsicle socials, manicures, musical performances, and plenty of other activities. Plus, the community offers a library, a garden, a game room, a screened lanai, a woodworking shop, and other amenities to enjoy. There are a variety of ways to stay engaged here while also having some fun. If your loved one needs assistance obtaining their personal items, such as toothpaste, toilet paper, floss, or other necessities, the staff can assist. Through the Personal Solutions program, staff members will deliver item requests to residents' front doors. Best of all, there is no extra charge for the delivery. Living Arrangements The carefully designed apartments here promote familiarity and the comfort of home. Residents can choose between a private or semi-private apartment to best match their lifestyle. The quaint spaces ranging from 285 to 404 square feet provide storage space and plenty of natural light. Positive[Name Removed] the nurse on the memory care unit is WONDERFUL. She had a personal approach. She knew how to help, interact and help my aunt. It went beyond doing a job. We had to change my aunt because she needed a higher level of care. My aunt never complained about the food. The breakfast looked appetizing and was sufficient for my aunt. Very clean Room size was efficient and big enough. Biggest complaint was getting return calls from[Name Removed] when i needed information. Unsure of her title.

PositiveWhen I compared with the [community removed], which cost the same amount, the Dr. Phillips location is so much more upscale and beautiful and location is better too for class of living and everyone has been extremely friendly. I have never had a call about a concern with mom since I moved her in [date removed]. I have gone to visit mom every couple of days since I moved her in and have been extremely happy with the kindness of all of the staff

Positive[name removed] was very nice and thorough in showing me all Brookdale offer’s for my Dad and Stepmom when needed. I was a little turned off by the expense, but now I understand that it is costly to take care of people. I want them to be happy and I’m trying to be proactive and find a place when they need it and yes, I believe Brookdale is a very nice place.

PositiveI visited a friend staying here. The place looked WONDERFUL all decorated for Christmas and everyone seemed so welcoming and happy. The only thing I noticed was the food didn’t look super appealing and they were joking about how bad it was. Hard to please everyone, but I didn’t feel like eating there to find out. The dining rooms are smaller very homey but with stains and dirt on the wall and floors so it’s hard to find that appealing while eating. Hopefully it was a one time thing but it looked like it hadn’t been…

PositiveMy loved one was welcomed kindly and warmly. They have SOME great nurses that know the resident individually and treats them based on their individual personalities. Dissemination of information across the entire team seems to wane at times. The facility is always clean. The residents look appropriately dressed and clean. They seem to want to make sure the residents are well taken care of.
