A Brookdale HealthPlus ® Community Brookdale's HealthPlus program also provides a higher level of care without higher costs. With a focus on minimizing hospitalizations and emergency room visits, Brookdale HealthPlus offers care coordination, preventive health care services such as annual wellness visits, immunizations, and health screenings, and support for managing chronic conditions such as diabetes, high blood pressure, and congestive heart failure. Residents benefit from care coordination overseen by a Brookdale HealthPlus Care Manager (RN). Community Services and Amenities Residents in assisted living have an abundance of engaging activities to look forward to including mix-and-mingle socials, live musical performances, movie nights, and off-site excursions to the Elks Lodge for bingo. For those in memory care, your day may be filled with fun brain games, singing, crafting, and other purposeful activities. Additionally, residents have access to a library, a therapy room, and an arts and crafts studio. In the state of Texas, these terms are defined as follows: Assisted Living Facility A facility that provides personal care services and administration of medication to 4 or more residents. Assisted living facilities can accept residents with dementia and/or Alzheimer’s. There are 3 types of assisted living facilities: Type A: A facility that may provide room and board to residents who are physically and mentally capable of evacuating the facility unassisted. Type B: A facility that may provide room and board to residents who need assistance evacuating, are incapable of following directions under emergency conditions, require staff attendance during nighttime sleeping hours, and require assistance transferring to and from a wheelchair. Type C (Adult Foster Care): Four-bed facilities that meet the minimum standards and rules for enrollment in the Department of Aging and Disability Services’ Adult Foster Care home program. Took Great Care of Mother In Law

My MIL lived at Brookdale Lake Highlands for right at 2 years and her care was exceptional. We originally placed her in the assisted living side, but it became apparently quickly that she needed memory care. The facility worked with us on this and was very accommodating along the way. In addition, Brookdale assisted us in securing Hospice to add an additional level of care for her. Wonderful care for my wife

I recently had to locate a place for memory care for my wife. After visiting with the staff of Brookdale Lake Highlands, I knew I didn't have to look any further. The staff was very caring and obviously had my wife's best interests at heart. The residents I saw during my visit all seemed very happy and content. That feeling was confirmed later as I was leaving after my first visit. I walked out with the daughter of a current memory care resident. When I told her I was thinking about placing my wife there, she said

A Good Community

4 only because of the food. I was very fortunate to have found Brookdale in such a short time. The facility is clean, the staff is very friendly and caring And they have a number of weekly activities such as bingo on MWF, a book club and occasional field trips to lunch or to Choctaw. There are groups who come regularly to perform or do other things. The memory care unit is outstanding with a very caring and knowledgeable director and staff. I met some great people there and go back to visit at least several times a

1. Overall experience- we made a decision quickly. Brookdale’s proximity to our home was a strong determinant. medical plan from my husband PCP delayed move in but Brookdale hooked me up with flexible movers. Move in was a bit erratic and thought there would have been a resident/family orientation - nope!
