Tucked into a quiet cul de sac, in a quaint ranch-style house, Aging Gracefully Family Home of Vancouver, Washington provides memory care services for up to six residents of any age. Our brick-accented home was built in 1993 and most recently renovated in 2013. Nearby parks and green spaces are perfect for an afternoon enjoying the aroma of evergreens and the energizing Pacific Northwest breeze. We’re located less than one mile from PeaceHealth Southwest Medical Center for any emergency or acute medical needs. Community Services and Amenities When we say “personalized care,” imagine activities and services tailored specifically for your loved one. We’ve been known to do up residents’ hair exactly as they please, assist in digitizing prized photographs while listening to the memories behind them, and pay particular attention to favorite activities. Our spacious living room is appointed for comfort and relaxation, featuring cozy recliners, a couch, warming fireplace, and a large TV. Houseplants liven up our indoor common areas and improve indoor air quality. Outside common areas are accessible for all to enjoy the nicely landscaped yard. We provide all meals and are happy to make adjustments for shifting dietary needs or preferences. We’re here to help residents maintain their health and wellness, with particular attention to cognitively stimulating activities. On any given day, you might find us piecing together a puzzle, getting in a workout on the exercise bike, or taking an off-site excursion for a change of scenery. Diverse medical services are available, including services specific to memory care. We offer transportation, and on-site resident parking. Living Arrangements We offer several styles of rooms, readily furnished to make moving a little easier of a transition. Personal decorations are encouraged to add flair and familiarity. As a single-story home, all rooms are conveniently located on the ground floor for added ease of mobility. Some rooms feature private bathrooms, and wheelchair-accessible showers are available. Care Types Offered Aging Gracefully Family Home is an Adult Family Home which provides Expanded Community Services . In the state of Washington, these terms are defined as follows: An Adult Family Home is a facility that provides assistance with daily activities, health support services, and intermittent nursing services for two to six residents. Health support services include dementia care and mental health care. Expanded Community Services (ECS) includes personal care services, medication oversight, and contracted behavior support services. Residential providers may offer increased staff or activities to support the client in the residence.
