CriticalThe facility was consistently under staffed. One time I called and only the receptionist was in the building. There was another staff member in the memory care side but not the ALF. The facility will not communicate or respond to simple questions. The facility does not notify you about injuries to residents. The facility has regular turnover in the executive director position and other staff. They do not notify the family point of contact. The facility did not keep the room clean.
