Meadow Falls of Middletown is an intimate Assisted Living and Memory Care community located in Middletown, Ohio. Nestled in a neighborhood surrounded by beautiful homes and churches, we are only five minutes away from downtown where you’ll find plenty of shopping, Meadow Falls of Middletown, and an array of casual and upscale restaurants. The staff at the Meadow Falls of Middletown are here to enrichen the lives of our residents in their home and promote their independence with love and dignity. We get to know our residents and their health needs, their hobbies, and their social desires. Our care plans are individually tailored for each resident and regular assessments are conducted to ensure proper care is being delivered. We have around the clock staffing including a nurse on location 24/7. For those residents that may need Physical Therapy or Occupational Therapy, we have on-site service providers. Not only does our community help those that need assistance with activities of daily living (ADLs), but we also help those with memory and cognition challenges along their journey. Our thoughtfully designed floor plans and convenient services give our residents the freedom to live a vibrant lifestyle without the typical day-to-day worries. Meadow Falls of Middletown provides three restaurant-style meals served daily, available housekeeping and laundry services, and on-site maintenance. We have two beautiful courtyards, one for Assisted Living and a secured courtyard for Memory Care. Both courtyards feature walking paths, trees, flowers, and comfortable sitting areas. And, of course, we have recreation and entertainment programing to help our residents be physically and mentally fit, while being socially engaged. PositiveMy parents lived there for almost 8 years. During their stay, we had many great nurses, staff, aides, and admin. Being there, being loved and cared for was such a blessing for them and me! PositiveFood, activities and friendliness is great. The pricing HAS inched up a couple of times and now they are introducing direct pay or pay $25 extra to pay by check. Not happy about this. I'm not sure how taking checks to the bank once a month would cost each resident $25. I hope they will reconsider this idea. I would recommend them to others.

PositiveI gave a 4 on Quality of care . But that was due to a Nurse that wasn't making sure my Sister got her meds. They have another Nurse & things r fine . It is hard I to have employees now days that want to work. But they have been great to my Sister & she loves it there. She calls it home . She just turned 86 [date removed]! Doing well for her age!
