A Brookdale HealthPlus ® Community The residents at Brookdale South Park can enjoy the benefits of Brookdale HealthPlus, which offers on-site care coordination overseen by a registered nurse. Here, residents receive personalized care plans and assistance with chronic conditions, such as diabetes, high blood pressure, congestive heart failure, and more. Preventative health care services, such as annual wellness visits, immunizations, and health screenings are also provided to help avoid trips to doctors’ offices, or unnecessary emergency room visits and hospital stays. At Brookdale South Park, the dementia-trained staff members understand there is not a one-size-fits-all approach to memory care. Here, each individual can expect a personalized experience. This means every resident receives a tailored care plan with consistent care assignments. There are two Brookdale memory programs available here: Clare Bridge and Solace. Although it was created more than 30 years ago, the Clare Bridge program continually evolves with new research, and the goal is to provide residents with six planned activities per day. These activities encourage residents to use their current skills, while preserving their abilities and true self. The Solace program does not replace any elements of Clare Bridge. Instead, it adds additional programming, such as music and aromatherapy, while providing assistance with bathing, dressing, and more. Rest assured, staff members are available 24 hours a day, seven days a week to be alerted to an emergency. Additionally, residents benefit from community safety features, including a 24-hour security system and keypad entries. Assistive physical features are also present on-site, such as large circular paths designed to lead residents to areas of activity, as well as way-finding cues, and spaces that are decorated to feel like home. Residents of Brookdale South Park can look forward to an easier lifestyle with less chores and stress. Homemade meals are prepared and served daily, and a variety of housekeeping chores are managed for you. Transportation assistance, postal services, and concierge services are also available, to assist residents as needed. Daily life here is designed to feel effortless! Community Services and Amenities Residents can participate in daily cognitive workouts, brain stimulating games, and B-Fit program activities like walking groups, dancing, and more. The community features a beautiful courtyard and garden, scenic walking paths, and an arts and crafts studio, which are all popular places to unwind. For convenience, Brookdale South Park offers an on-site beauty and barber shop, as well as a therapy room, to be used by residents as needed. If you need assistance obtaining personal hygiene items, such as toothbrushes, toilet paper, floss, or other necessities, be sure to let the staff know. Through the Personal Solutions program, staff members will happily deliver your item requests to your door. Best of all, there is no delivery fee!
